The Hemlock Society of Florida is offering free, downloadable end-of-life forms on a new website.
The organization announced the website on Monday.
Florida-specific living wills do not resuscitate orders, and health care surrogate designations are intended to make sure people's wishes are followed in case they became too ill to express them.
The site includes instruction on where to keep the forms on file and warns that do not resuscitate orders are not valid under Florida law unless printed on yellow paper.
The group's executive director, Ben Wilcox, said he hopes the website will help Floridians start conversations with family, friends and doctors about their end-of-life wishes.
